A scaling device used in health measurement. A common form, in which the respondent is invited to rate an entity (like 'health' or 'satisfaction') on a scale is to circle a number:
Good 3 2 1 0 1 2 3 Bad
where 0 is neutral and one moves outwards from it towards the extremes of goodness and badness (of 'health' etc.).
